I know it's actually quite hard to find a complete list of every T5 skin on the internet, so I'm mainly making this as an archival repository, for anyone looking for this information, present or future. Please do let me know if there are any details I've missed for any of these skins.

It's worth noting that a Tier 5 skin is defined as a skin in which the god's entire model, animation set, ability effects, and voice pack are replaced, essentially requiring the same amount of artistic effort as a new god's base model, if not more.

Each of these skins has an animated card art, which is a unique feature they all share. For skins with multiple forms, each form also has its own voicelines.
